I already did. The response I got was 'Kindly submit the original form listing the driving offences, infringements or convictions'. It's the same as what's written on the whp canada website. Does this mean just the third sheet with the traffic offenses? It doesn't look very authentic though since there's no letterhead on the printout. It's quite basic.

Thanks Jared. Took your advice of scanning a copy of the form and emailed them again. They have said to include the attached form (traffic offences) and the demerit point extract form. Glad I finally have a definitive answer. Didn't want to send off the wrong form and have to resubmit it. Thanks for your help.

Thanks Anna. I already sent off my application. I only included the 2 forms they asked for in the email. I really wanted to keep the first one with my license start date for when I apply for my license in Alberta. I wish they hadn't added this, it was so much easier the first time I applied.

This post was so useful. I have been fighting with VicRoads for the past two weeks. Since they include the 5 year demerit point extract I have been arguing with them that this is not the full history report. They have sent over the same information twice. I guess it's correct...

Oh well, at least I have a spare copy now.
